FY 2024 BEHAVIORAL HEALTH SERVICE EXPANSION - EXPANDING ACCESS TO BEHAVIORAL HEALTH AND SUBSTANCE USE DISORDER TREATMENT IN CENTRAL GEORGIA FIRST CHOICE PRIMARY CARE PROPOSES TO INCREASE ITS CAPACITY TO PROVIDE NEEDED BEHAVIORAL HEALTH SERVICES AND INITIATE A PROGRAM OFFERING MEDICALLY ASSISTED TREATMENT TO RESIDENTS OF OUR SERVICE AREA. WE WILL WORK IN COLLABORATION WITH AN EXISTING MAT PROVIDER, RIVER EDGE BEHAVIORAL HEALTH SERVICES. RIVER EDGE AND FIRST CHOICE HAVE A LONG STANDING COLLABORATIVE PARTNERSHIP, AND WE WILL LEVERAGE THEIR EXPERIENCE TO REACH A BROADER SEGMENT OF THE COMMUNITY, ADDRESSING UNMET NEED IN A COMPREHENSIVE FASHION. FUNDING FROM THIS OPPORTUNITY WILL BE USED TO HIRE A FAMILY MEDICINE PROVIDER WITH AN INTEREST IN MAT. THAT FM PHYSICIAN WILL DEVOTE TWO DAYS PER WEEK TO THE MOUD PROGRAM, AND AS THE PROGRAM GROWS THAT TIME COMMITMENT MAY INCREASE. FUNDING WILL ALSO SUPPORT AN ADDITIONAL PART TIME PSYCHIATRIC NURSE PRACTITIONER, AND INCREASE THE CONTRACTUAL TIME FOR A PSYCHIATRIST TO SUPERVISE THAT NP. WE WILL HIRE AN ADDITIONAL LCSW AND A SUBSTANCE USE DISORDER COUNSELOR TO PROVIDE SUPPORTIVE SERVICES, INCLUDING GROUPS. WE WILL ALSO ADD 1.0 FTE OF A REGISTERED NURSE WHO WILL MANAGE THE MOUD PROGRAM DAY TO DAY. THE INITIAL SERVICES WILL BE PROVIDED AT OUR LARGEST CLINICAL SITE IN DOWNTOWN MACON, AND IN YEAR 2 WE WILL ADD SERVICES IN OUR WARNER ROBINS OFFICE, WHICH WILL EXTEND SERVICES TO ANOTHER COUNTY IN OUR SERVICE AREA. THE TWO COUNTIES IN WHICH WE CURRENTLY OPERATE ARE SURROUNDED BY SMALLER RURAL COUNTIES THAT ARE NOT ABLE TO OFFER MANY BEHAVIORAL HEALTH SERVICES, SO WE ANTICIPATE OUR PROGRAM WILL BE ABLE TO REACH RESIDENTS OF THESE COUNTIES AS WELL. YEAR 2 EXPANSION TO WARNER ROBINS (HOUSTON COUNTY) WILL REQUIRE ADDITIONAL NURSE PRACTITIONER TIME, AS WELL AS ANOTHER PART TIME COUNSELOR. WE OPERATE PHARMACIES IN BOTH LOCATIONS THAT WILL ASSIST IN MANAGING THE NECESSARY MEDICATIONS IN A SAFE AND COMPLIANT FASHION.

PURPOSE: THE EMERGENCY SOLUTIONS GRANT (ESG) AUTHORIZES THE DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT (HUD) TO MAKE GRANTS TO STATES, UNITS OF GENERAL-PURPOSE LOCAL GOVERNMENT, AND TERRITORIES FOR THE REHABILITATION OR CONVERSION OF BUILDINGS FOR USE AS EMERGENCY SHELTER FOR THE HOMELESS, FOR THE PAYMENT OF CERTAIN EXPENSES RELATED TO OPERATING EMERGENCY SHELTERS, FOR ESSENTIAL SERVICES RELATED TO EMERGENCY SHELTERS AND STREET OUTREACH FOR THE HOMELESS, AND FOR HOMELESSNESS PREVENTION AND RAPID RE-HOUSING ASSISTANCE. THE ESG PROGRAM IS A FORMULA GRANT PROGRAM THAT ASSISTS INDIVIDUALS AND FAMILIES TO QUICKLY REGAIN PERMANENT HOUSING STABILITY AFTER EXPERIENCING HOMELESSNESS OR A HOUSING CRISIS. CURRENT ESG AWARDS AND ALLOCATIONS ARE INCLUDED IN THIS LIST OF HUD AWARDS AND ALLOCATIONS LOCATED AT HTTPS://WWW.HUD.GOV/PROGRAM_OFFICES/COMM_PLANNING/BUDGET/FY23; ACTIVITIES TO BE PERFORMED: ESG FUNDS MAY BE USED FOR FIVE PROGRAM COMPONENTS: STREET OUTREACH, EMERGENCY SHELTER, HOMELESSNESS PREVENTION, RAPID RE-HOUSING ASSISTANCE, AND HOMELESS MANAGEMENT INFORMATION SYSTEM (HMIS). THE BIBB COUNTY SHERIFFS OFFICE RESPECTFULLY SUBMITS THIS APPLICATION FOR THE BUREAU OF JUSTICE ASSISTANCE FY24 EDWARD BYRNE MEMORIAL JUSTICE ASSISTANCE GRANT (JAG) PROGRAM. THE SHERIFFS OFFICE REQUESTS FUNDING OF $155,706 TO ADDRESS VIOLENT CRIME IN THE MACON-BIBB COUNTY COMMUNITY. THE PROJECT WILL UTILIZE TECHNOLOGY, TRAINING, AND EQUIPMENT TO ENHANCE PROACTIVE POLICING AND INVESTIGATIVE CAPABILITIES. LOCAL STATISTICS DOCUMENT THE SIGNIFICANCE OF THE PROPOSED PROJECT. WITH 2,698 VIOLENT CRIMES COMMITTED IN 2023, THE COUNTYS VIOLENT CRIME RATE PER 100,000 RESIDENTS OF 1,724 REPRESENTS MORE THAN FOUR TIMES THE NATIONAL AVERAGE OF 380. DEMOGRAPHIC AND ECONOMIC STATISTICS DOCUMENT THE ADDITIONAL CHALLENGES FACED BY THE COMMUNITY. THESE INCLUDE POVERTY RATES MORE THAN DOUBLE THE NATIONAL AVERAGE, A POPULATION PREDOMINATELY HIGH RISK, AND A GEOGRAPHIC LOCATION PROVIDING CONVENIENT ACCESS TO HIGH INTENSITY DRUG TRAFFICKING AREAS. WHILE THE PRIMARY TARGET AREA INCLUDES THE LOCAL COMMUNITY, THE PROPOSED TECHNOLOGY AND EQUIPMENT WILL ALSO ENABLE THE SHERIFFS OFFICE TO MORE EFFECTIVELY COORDINATE INFORMATION WITH OTHER JURISDICTIONS AND AGENCIES TO ADDRESS CRIME BEYOND MACON-BIBB COUNTYS BOUNDARIES. MONITORING OF CRIME STATISTICS WILL PROVIDE PERFORMANCE MEASUREMENTS TO GUIDE DECISION MAKING AS THE PROJECT PROGRESSES AND TO DOCUMENT THE EFFECTIVENESS OF THE PROJECT. ANALYSIS WILL INCLUDE COMPILATION OF LOCAL STATISTICS AND COMPARISONS TO HISTORICAL DATA AND SIMILAR DATA ON THE STATE AND NATIONAL LEVEL. THE BIBB COUNTY SHERIFFS OFFICE REPRESENTS THE DESIGNATED LAW ENFORCEMENT AGENCY FOR MACON-BIBB COUNTY. SHERIFF DAVID J. DAVIS WILL ENSURE ADEQUATE, QUALIFIED STAFFING TO SUPPORT IMPLEMENTATION AND MONITORING OF THE PROJECT. THE BIBB COUNTY SHERIFFS OFFICE CONSISTENTLY RECEIVES FEDERAL FUNDING FOR A VARIETY OF PROGRAMS AND UNDERSTANDS THE COMPLIANCE AND REPORTING REQUIREMENTS ASSOCIATED WITH FEDERAL PROGRAMS. THE SHERIFFS OFFICE WILL COMMIT THE RESOURCES NECESSARY TO ENSURE THE SUCCESS OF THE PROJECT. UTILIZING DATA AND INNOVATIVE APPROACHES PROVIDES EVIDENCE-INFORMED STRATEGIES TO REDUCE CRIME THROUGH TAILORED, COMMUNITY-CENTERED INITIATIVES. MULTIDISCIPLINARY STRATEGIES ENGAGE INDIVIDUALS AND GROUPS TO PREVENT AND DISRUPT CYCLES OF VIOLENCE AND ESTABLISH RELATIONSHIPS BETWEEN INDIVIDUALS, LAW ENFORCEMENT, AND COMMUNITY PARTNERS TO DELIVER SERVICES THAT IMPROVE THE PHYSICAL, SOCIAL, AND ECONOMIC CONDITIONS THAT DRIVE VIOLENCE. THE SHERIFFS OFFICE WILL UTILIZE THE RESOURCES PROVIDED WITH THIS FUNDING TO IMPROVE INVESTIGATIVE CAPABILITIES WITH THE GOALS TO PROMOTE CIVIL RIGHTS AND RACIAL EQUITY, INCREASE ACCESS TO JUSTICE, SUPPORT CRIME VICTIMS AND INDIVIDUALS IMPACTED BY THE JUSTICE SYSTEM, STRENGTHEN COMMUNITY SAFETY, PROTECT THE PUBLIC FROM CRIME AND EVOLVING THREATS, AND BUILD TRUST BETWEEN LAW ENFORCEMENT AND THE COMMUNITY.
